What is Platelet Rich Plasma and MicroDermal Needling?(*)
The outcome of the MicroDermal Needling Facial procedures smooths out facial wrinkles, stimulates collagen production, thickens one’s skin, improves the skin’s blood supply (microcirculation) and rejuvenate your complexion.

How does the MicroDermal Needling or PRP work?
Dr. Troell improved the classic description of the MicroDermal Needling or PRP procedure by removing the skin barrier to absorption of the PRP. Adding microdermabrasion and dermal needling using the MicroPen enables better absorption of the platelet rich plasma that is applied topically (Facial) or through intradermal injection (Facelift).
Many patients can perform the procedure with topical anesthesia for thirty minutes if the dermal needling is being performed or only with the anesthetic in the dermal filler if the dermal needling is not added to the procedure. Some prefer the addition of sensory nerve blocks, each one injecting one millimeter (1/5 tsp) of local anesthetic into each sensory nerve of the face (trigeminal nerve). Also, some patients prefer to relax with oral sedation with Ativan (lorazepam), similar to valium, and a narcotic analgesic, hydrocodone (norco, vicodin, lortab). These are rarely needed. The discomfort from the MicroDermal Needling procedure is minimal with no patients needing pain medications after the procedure.
After the patients face is cleaned, the patient in a relaxed state lying in a comfortable examination reclining bed, and the local anesthetic completed, the first step of the MicroDermal Needling is carried out.
The facial volume loss or deficiency may be temporarily improved by the addition of a dermal filler. The dermal filler options include those of hyaluronic acid based fillers such as, Juvederm, Perlane and Restylane.
The epidermal superficial skin, superficial and deep dermis (papillary and follicular) and underlying subdermal mantle fat layer complete the skin structure lying over the animating facial muscles. Juvaderm is placed into the deep dermis and Radiesse is placed under the dermis in the superficial mantle fat layer.
Injecting these substances with a specialized blunt cannula, DermaSculpt or Magic Needle, instead of the needle that comes with the dermal filler lowers the risk of errant injection, makes the plan of injection more homogeneous and lowers bruising.
Alternatively, fat transfer alone or augmented with your own stem cells and regenerative cells may be chosen to provide a permanent volume enhancement of the facial volume deficiencies.

The process of accessing your stem and progenitor cells begins with a small area of fat removal (liposuction) using ultrasound (VASERlipo) under oral sedation and local anesthesia. The fat cells are harvested with a 85% fat cell survival rate and an 87% stem cell survival rate with this method.
Once these cells are harvested, they are processed in a revolutionary closed centrifuge system, known as the LipoKit or Adivive system. The Adivive fat processing unit has yielded the most pure and compact fat and stem cell cellular components.
The syringe containing your fat and stem cells, separated by free oils and excess blood and wetting solution fluid, is then processed in the Maxstem/Celltibator to concentrate the stem and progenitor cells. This stem cell processing unit increases the stem cell concentration from 2-5% in the fat layer to nearly 100% pure stem and progenitor cells.

These stem cells have been shown in a Stanford University study to keep their capabilities to become any cell of your body, however, they probably become blood vessels (endothelial cells and perivascular cells) and adipocytes (fat cells).
During the MicroDermal Needling, Dr. Troell will create your own Platelet-Rich Plasma (PRP), which is a form of blood plasma that has been enriched with platelets and growth factors. Platelets are tiny cell fragments that stop bleeding and bruising and contain growth factors and natural substances that encourage cellular growth. They provide you with the building blocks for healing when you sustain a burn or cut.
The PRP is acquired by placing a small cannula in your arm vein, drawing out four teaspoons of blood into two special test tubes. The test tubes are spun in a centrifuge, separating the platelets and growth factors from red blood cells by the most efficient patented resin available.
The special patented polymer to concentrate the platelets and growth factors is the best on the market.
The PRP, along with the facial filler, Juvederm, is then injected back into the face. This method of combining Juvederm and PRP dramatically enhances the effects of the treatment because the platelets contain important growth factors that ignite tissue regeneration, improve the blood supply or microcirculatiion and stimulate collagen production.
What is a MicroDermal Needling procedure like?
The MicroDermal Needling is a minimally invasive procedure, similar to the procedure of administering a dermal filler treatment. The procedure begins with drawing a vial of blood from the patient. The provider places the blood in a centrifuge and spins the blood at a very rapid rate to separate the platelets from the other components of the blood. The platelets are then activated by Calcium, producing your very own Platelet-Rich Plasma (PRP).
After administering the dermal filler, Dr Troell uses the smallest needle (30 g) to ease the PRP under your skin to fill the soft tissue of the dermis. A “Global Facial Injection Technique” can be employed that reaches all the facial areas. This process adds volume, diminishes the appearance of wrinkles and the appearance of the nasolabial folds, improves the skin’s appearance and stimulates the production of collagen, with minimal downtime.
What are the benefits of the MicroDermal Needling?
