Thank you very much for your inquiry regarding breast augmentation surgery. There are two techniques of breast augmentation: 1. natural breast augmentation, harvesting one’s own fat elsewhere in the body, processing the fat for purification and fat grafting to the breast, and 2. breast implant surgery using either a saline or silicone implant.
I completed a residency and fellowship at Stanford University Medical Center and remained there as a clinical professor for seven years. I am quadruple board certified including the American Board of Cosmetic Surgery in Breast, Body & Extremity. I am founder of the Institute of Advanced Body Contouring. I have a practice in Las Vegas, Nevada. I have presented fourteen live surgical seminars, teaching other cosmetic surgeons the art of body contouring including liposuction, natural breast augmentation, Brazilian Butt Lift and tummy tucks. Currently I am Adjunct Professor of Touro University, College of Osteopathic Medicine.
Harvested body fat can be used to provide breast augmentation or enhancement using one’s own fat, the outcome is a natural appearing breast that is soft without the artificial appearance of implants. Usually 150-240 cc of fat is placed in each breast, which augments the size by one or two cup sizes. The fat can also reshape asymmetries in the breast. I am a national trainer for SlimLipo laser lipolysis, the water-jet assisted liposuction (Body-Jet) and VASER technologies as adjuvants in liposuction.
The fat grafting methods that I am using are currently regarded as the best techniques for fat grafting, increases the fat survival up to 80% of the grafted fat, compared to only 40-60% with other conventional techniques. Dr. Robert J Troell, FACS, FAAFPRS, FAACS, FAAOHNS, FAASM has recently completed a study on breast fat grafting comparing two different methods for fat processing and purification presented at the World Congress of Liposuction 2013.
Breast implant surgery is the most common cosmetic surgery performed in the US, with liposuction being the second. With current surgical technologies, the procedure is safe and effective.
The decision to use a saline implant versus a silicone implant is related to patient preference and the amount of fatty and breast tissue currently present. Implants are usually placed below the chest (pectoralis) muscle, however, in woman with significant atrophy from pregnancy, above the muscle or subglandular placement may be preferred.
Silicone implants offer a more natural feel and appearance with less chance of rippling. Incision locations include: around the nipple (Infra-areolar), in the anterior axilla fold (trans-axillary), superior aspect of the belly button and below each breast (inframammary). Some patient’s also have breast droopiness (breast ptosis) that requires a simultaneous breast lift (mammopexy) to optimize the final appearance.
