Thank you very much for your inquiry regarding cosmetic lip augmentation. I have 20 years of experience performing lip augmentation. I completed a residency and fellowship at Stanford University Medical Center and remained as a clinical professor for seven years. There are both temporary and permanent methods to either enlarge (augment) or show more mucosa of the upper and lower lips. The options include: skin (dermal) fillers, fat grafting, pericranial graft placement, soft silastic implant and lip advancement flaps.

I have published a book chapter in a Stem Cell textbook of aesthetic medicine that I have presented at several cosmetic surgery meetings on the use of fat grafting (Naturalfill) for lip enhancement (as well as using fat for all body areas). The limitation of fat is that more than one procedure is necessary for adequate size, although the fat can be stored.
The most common permanent technique is a silastic implant that can be inserted into the lip called Perma Lip. A tapered soft silastic implant of varied thicknesses and lengths allows minimal traumatic placement. Patients have been very responsive to this technique with minimal discomfort and rapid recovery with no infections or extrusions to date. Size implants vary from 3, 4 and 5 mm diameters and tapered lengths 55, 60 and 65 mm. All of these permanent lip augmentation procedures can be performed under local anesthesia.
Temporary lip enhancement can be achieved by injecting hyaluronic acid dermal fillers, such as Perlane and Juvaderm under local anesthesia. The results with dermal fillers remain between 4-12 months, thereafter, additional injections are necessary.
